- Birth Certificate (Original or Certified copy)
- Utility Bill (Electric, Gas, City Water bill only NO other household bills can be accepted)
- Report Card (First registration report card my be brought to first practice)
- Sports Physical (must be on Pop Warner form and Physical cannot be performed by RN per AZ State Law)
- Parental Consent Form (Signed by Parent and Participant)
*The Parent that Signs up the participant must be on Birth Certificate and Utility Bill for verification only.
*If utility bill does not match the parents name, a School Verification form can be used instead of a utility bill.
*All items above must be turned in order for each child to participate
